INCIDENT REPORT

All umpires who wish to file an incident report regarding an ejection must complete the form below. The form will be submitted to the Baseball Alberta Umpire Committee as well as the Baseball Alberta Discipline Committee for review.

The Umpire Incident Report is the formal mechanism used by Baseball Alberta to document ejections, significant game incidents, and behavioural concerns that occur during any Baseball Alberta sanctioned game or tournament. These reports support consistent followup, help maintain a safe and respectful environment, and ensure that officials are backed by clear, factual records of on‑field situations.

Umpires can access the form directly through the link provided below on this website. After completing the report, the information is automatically submitted to Baseball Alberta for review by staff and the Provincial Supervisor of Umpires.
